Updated the main executable to help address some application startup issues:

• Add logging during application startup in order to diagnose startup failure issues.
  • Look for startup.log within the My Documents/My Games/Double Helix folder.
• Updated the graphics library to use the adapter corresponding to the selected desired refresh rate.
